Debt collection disclosure verification of debt not given enough info to verify debt June 19, 2016 - I have asked PDQ Services Inc to validate a collection they are reporting on my credit reports on -/-/16, -/-/16, -/-/16, -/-/16 and they have failed to provide anything that would support the fact that they are attempting to collect on a debt without an agreement or contract. Nor have they supplied anything other than a statement on - of the collections. I told them that I would be reporting them to you but they obviously do n't fear you because they failed to delete themselves or answer questions about the debt ( s ) that they are using to damage me financially. PDQ Services, Inc. Debt collection disclosure verification of debt right to dispute notice not received December 25, 2015 - I went to the doctor for a medical condition with -. I completed all of the necessary paperwork and the doctor 's office filed a claim on my behalf which was supposed to have been sent to the insurance company ( - ). I never heard anything back from the doctor 's office after that nor did I get any information from the insurance company stating that I owed any money. Shortly after that visit I moved to a new address and never received a bill from the insurance company or the doctor. The doctor 's office 's posted financial policy, that I agreed to, states only the custodial parent will be held responsible for any unpaid balances. In addition, the State of Georgia has a Parental Responsiblity Act reflecting a law which states only the custodial parent can be held responsible. - - - says, since I filled out the medical forms, it is assumed that I am the parent, therefore have joint responsibility.
