I sent Equifax - requests to place a Security Freeze on my file. The First request was sent on -/-/16, the Second on -/-/16 and the Third request on -/-/16. Each request was sent certified mail and included my complete name, complete address, Social Security number and date of birth. These request also included a copy of my Drivers License, -, Proof of address and my Identity Theft Police report. I also sent the same request for my wife - with the same documents - and she was able to obtain the security freeze upon the first request.
