Medical Cont'd attempts collect debt not owed Debt was paid Texas

I received letter from Collection agency called " Capital Accounts '' and then received a phone call at my workplace. I did not receive any letter, bill or any kind of communication from the Dental service provider " - '' since - 2012. The services were rendered in - 2012. I paid over $1500.00 Cash and rest was supposed to be paid by my insurance. According to Capital Accounts I owe them around $2300.00 According to " - '' I owe them around $1500.00. Why - - not contacted me ever and I received a sudden letter from collection agency after 45 months. I believe that either I do n't owe them anything or - failed to file claim with Insurance Company in timely fashion and now trying to make me pay for their mistake Or there are some kind of fraudulent activities going on somewhere in the pipeline since - Office never contacted me about any Insurance Issue at that time and now 45 months have passed. I would appreciate if you can step in and investigate since they are threating to destroy my credit with Credit Bureau. Thanks
