Incorrect information on credit report Personal information California

After having my - credit card number stolen ( attempt to charge online fraudulently on -/-/- ), I closed my account and received a new card number. On -/-/2016, I pulled my credit report and saw an incorrect ( fraudulent ) address listed and a - credit inquiry filed on -/-/2016. I disputed the fraudulent address with Experian, placed a fraud alert on my credit file, and contacted - that an account in my name but new address was fraudulent which they then denied. The incorrect/fraudulent address ( -, - OHIO - ) is a homeless shelter called " The - ''. I want to file a police report re : this fraud. I am now concerned about how much of my personal information has been compromised and am concerned about the extent of my identify theft and safety of my credit file.
