Other (i.e. phone, health club, etc.) Disclosure verification of debt Not given enough info to verify debt New Hampshire

Medical services were provided in - 2015. Those services were put on a payment arrangement. I did not know that the 3rd party vendor was not a part of those payment arrangements, so it went to collections. The clinic forwarded the wrong mailing address to the 3rd party, so no bills were ever received by me. The collections agency called me -/-/- ( used my maiden name ), then called -/-/- ( by my married name ). We determined that I had never received any bills from their client or from them. I requested official copies of the bills, and was told I would not be reported to collections during the process, however they reported me in -/-/-. I did not get anything in writing from the collections agency until I called again in -/-/-. Finally in -/-/- I was sent the copies of the bills, which was the first time I had received any formal written documentation from the collections agency regarding the debt. I was unable to obtain bills for the services from the client they represent because it was sent to collections. -/-/- I paid the bill, and they insisted they had not reported me to the credit agencies. At that time they stated they would expunge the debt, which still has not been done.
