VA mortgage Loan modification,collection,foreclosure Georgia

We received a modification in -/-/- 2014. I was laid off of my Job -/-/- of the same year. We applied for another modification Through our servicer USAA. We were denied and then appealed it because of missing information ( copy inclosed ). We were told that per VA guidelines we are only allowed - modification every 3 years and only - during the life of the loan. Please assist us with this. Thank you
