I retired from my job at the age - where I had an HSA account. I was later contacted by Chase - operations that I had to fill out a form to get my - balance sent to me since I no longer worked at the company. Twice I called and twice I did not get the form, so I have yet to get my balance, approximately $3000.00. I am - and on social security. I need these funds returned to me.
