Incorrect information on credit report Account terms Georgia

I requested this company provide me with sufficient documentation to support this debt indeed being mine. I requested a copy of all documents ( invoices, summary of receipts, payments, etc. ) The company sent the attached document which is a mere form sent to my insurance company. I have never did any business with this company nor have I ever heard of them. This item is erroneous and should not be reported on my credit report. Delete immediately.
