U.S. Bancorp Credit card department,
Identity theft / Fraud / Embezzlement New York
A card was taken out with US Bank without my knowledge or authorization. I have contacted them on several occassions but they have yet to resolve this matter. I notified them that I am a victim of identity theft and that account is not mine.
