I went to see - - - - in 2013, to get -, on my first visit was the - exam and how much my - would cost me per -. I give them my - insurance card and a voucher of $120.00 for my - or -. I was told that my exam was $120.00, I let the lady at the front desk know she can use my $120.00 voucher to pay for the exam and - will take care of the remaining balance. I told the doctor I would have to come back for my -, do to money problems. The doctor told me to use the - trials for two weeks to see if I would be happy with them. Once my two week was up, I went back to order the - that 's when I was told I had a outstanding balance. Of which I never received any - nor - from them, I only received an exam.
