I received a - Form - from Department Stores National Bank, - -, SD for default on a Macy 's Credit Card. I sent a letter on -/-/- asking for a corrected - showing this was cancelled ( copy attached ). I have never had a Macy 's Credit Card. On -/-/-, I received a letter from the IRS ( copy attached ) and subsequently sent a letter to the Payer 's Name listed in the IRS letter asking for a corrected -. The letter was returned undelivered. The IRS is billing me f for taxes on this bad debt which does not belong to me. I want a cancelled - clearing me of this debt that I do not owe along with a copy of the credit application showing where, when and by whom an account was opened in my name.
