Incorrect information on credit report Account terms New York

I had a student loan from - - from -/-/-. This loan is paid off and closed on -/-/-. There was no late on this account. - and - never reported any late for this account.But on Equifax file a 150 days late showing -/-/-.The previous 4 months starting from -/-/-- -/-/-, it shows no late.All of of sudden a late of 150 days showing for -/-/-.If a student loan is late for more than 120 day, it goes into default status but in this case there was no default reported.If previous 4 months showing no late or payment made on time then how would it be 150 days late in just one month. Equifax has no answer for that and every time I dispute the account, they report back as verified.
