Incorrect information on credit report Information is not mine North Dakota

My credit report claims I opened a " Authorized Revolving Account on a FLE-IBLE SPENDING CREDIT CARD '' from - - -/-/- and that $25000.00 was spent on it between -/-/--/-/- which I had never put in for nor even heard of until I seen it on my report today. I do n't have a clue as to who opened this account nor where it got sent too, used at, etc. and would like a full report on this ASAP
