Incorrect information on credit report Information is not mine New Jersey

I have requested Chex Systems on numerous occasions to remove the the reported information from - - - - and each time it is removed and then re-instated. I have sent a police report, and an FTC Affidavit as these accounts were opened without my authorization. Please remove all inquiries from this credit union and remove the reported information again.
