Chase United Visa sent a fraudulently requested card to an address in -, Massachusetts without confirmation from us first. The Chase Visa representative said that the phone number the criminal was calling from showed up as my cell phone number and they used my husbands name and bank where payments are sent from. Chase did not ask any other security questions.
