Incorrect information on credit report Reinserted previously deleted info New York

Equifax is reporting a medical debt on my name under collection account in the name of " - '' in the amount of $440.00. It was deleted off my credit file in -/-/- because the collection agency failed verify the debt in 30 days. But Equifax reinserted the file in -/-/- and since then refusing to investigate the account even though this account is closed and ca n't be verified.
