Incorrect information on credit report Reinserted previously deleted info Wisconsin

On - -, 2016 I disputed an account that was on my report from - - - with all - Bureaus - All the Bureaus removed the data except - On -/-/-16 I received an updated credit report from Equifax ( see attached ) stating that the item had been removed. On -/-/-16 ( see attached ) I pull my report and confirmed it was removed. On -/-/-16 I received a notice that the item had been reinserted on -/-/-16 ( see attached ) and on -/-/-16 I pulled my report and confirmed that the item had clearly been reinserted. This is a violation of the FDCRA and I need your help.
