I have a victim of identity theft. My identity theft is well documented. Several years ago I banked with - Bank. - Bank had a data breach in which my name, social security number, driver 's license and date of birth were mailed to several - people. I am attaching a copy of a letter from - Bank which outlines the data breach and includes a toll free number for you to call to verify the claim of identity theft directly with the financial institution. My Chex Systems credit score has several factors which dictate the score. - of the factors are the addresses listed as well as the inquires from other financial institutions. The following inquiries were made without my consent or knowledge. I confirmed with several of the companies listed that the inquiries were made online and not by me.
