Credit card Disclosure verification of debt Right to dispute notice not received California

I have requested many times to see the letter ( with date and address ) to which the original letter was sent. I have never received this. Also, I have disputed this item many times with the credit bureaus as LVNV continiues to improperly report this debt. This debt is a collection. Not an open account and should be reported as such. Also, they continue to report the date of first major delinquency incorrectly. This has been going on for far too long and is wrong. I have attached a few smapled of the letters I sent to the CB. Each time LVNV returns with more inaccurate information. Like now. Please help.
