Medical Cont'd attempts collect debt not owed Debt was paid Texas

I went to an emergency room and paid the $150.00 required along with my insurance. I only found out that they had put something in to collections when looking at my Credit report. I received no phone call or letter with the itemized bill, which of course they should n't have been - seems my insurance paid for it. They have no number for debt collection or disputes of billing. I refuse to pay it considering I have no written letter or email or agreement to pay any bill. My insurance would have covered it. Thank you.
