Medical Disclosure verification of debt Right to dispute notice not received Missouri

A third party collection company is reporting an alleged debt to the credit reporting agencies. They never sent a notice of attempt to collect or right to dispute, nor any communication whatsoever. I only learned of the activity while checking my credit score. Also, they are reporting the same alleged debt - and attempting to collect - for the same alleged debt, which is related to a medical bill which was previously paid by insurance then reversed recently after my former employer illegally rescinded the policy and retroactively cancelled coverage by more than a year. The matter is currently under investigation by the EEOC, USDOL, - and pending civil litigation.
