I have requested proof of the debt they claim that originated in 2012 in from a medical visit at - - -. I have advised them I had insurance via - - in which I would have never visited a medical group outside the - - during that period. I would like proof of the medical debt reflecting my signature authorizing services faxed to - in which I have requested several times.
