Non-federal student loan Can't repay my loan Can't decrease my monthly payments Ohio

My loan repayment is extremely high with high interest rates. Repaying the loan is difficult with my salary I am currently collecting. Trying to talk to them to lower my monthly payments is a struggle, and forbearance actually hurt me when I tried it once. They still charged me after I fell behind on my loans and needed assistance by placing me in a forbearance for - months, where I still paid close to $500.00 per month.
