These are bills that were from having - take me to the hospital and for treatment of my back. This happened more than once as seen by the differant dates. I was found to be - with - and was put on - -/-/2011. I recieved medicaid at that time. I also filled out a financial hardship form from the hospital that promised to pay most if not all of the bills. I submited the bills to medicaid too. They were suppose to pay bills up to two years before I was put on medicaid.
