-/-/-, I received medical services from - -. On -/-/-, I paid the debt via my - credit card ( see endoscopic - ). On or around -/-/- I was contacted from the billing office stating that they had not received payment ( although it was made via credit card on -/-/- ). Since that time I have contact my credit card company ( - ) and supplied - - with copies of the relevant statements, including authorization numbers, claim number, etc. to no avail. On or about -/-/-, the bill was turned over to Fox Collections. I have spoken with - Fox representatives and supplied them with the relevant information and they have chosen to contact TransUnion and issue a non payment derogatory remark on my credit report. Fox collections contends that the provider was not paid because there is a 'denied amount ' indication on MY - statement ( - - ), when in fact this denial was NOT denial of the charge to the provider, but a denial of my - claim which I was required to satisfy myself with -. In fact, if this was a denial to the provider, they would be able to provide
endoscopic anesthesia.txt '' document outlining all the transaction details including the E-ACT date, time and authorization number of the payment ( provided by payflex credit card company ). The - '' document which is MY copy of the original - statement showing the - transaction. The Fox Collections.pdf '' document is a copy of the collections agency letter.
