I bought a $200.00 prepaid Master Card issued by U.S. Bank on -/-/2016. Before using the card in -/-/2016, I checked online to confirm the balance and saw that the card has been fraudulently used at - 's on -/-/- for $190.00. Since, the card has always been kept locked in my custody, I promptly called customer service on -/-/2016 and told them about this issue. The agent took my complaint and said that they will do a review. After about 15 days, I received a form which asked me to certify that the transactions disputed were not made by me. The letter said that filling up the affidavit ( form ) and sending it back was optional. However, to expedite the process, I filled it up and faxed it back to - on -/-/2016 at - AM -.


per the language on the form, it was optional anyway. Since, the conversation was n't going anywhere, the agent suggested that she can resend the form to me and I can fill and send it back to them again. I received the form again in a few days which I faxed back to the required number very next day. Unfortunately, today, I received a letter from - services dated -/-/2016 saying they ca n't help me with the disputed transaction since I reported it beyond the 120 day limit. I do n't agree how this issue has been handled due to following reasons : - I disputed transactions on -/-/2016 and -/-/- ( again ) both of which fall in the 120 day window begin quoted by - -. - - - conveniently denied receiving my fax on -/-/- even when I have a receipt showing it was successfully delivered. Hence, I strongly believe that - -. Bank are giving me a runaround for no fault of mine.
