Medical Disclosure verification of debt Right to dispute notice not received Ohio

This debt collection agency never notified me of the negative information that they reported. I contacted them via phone regarding the best and they refused to have it removed being that I was never notified of any negative information that they reported.. This Debt was paid on -/-/2015, Experience - claim they investigated and as of -/-/2015 the debt was still unpaid which is totally inaccurate. I paid this debt with a Mastercard and there is no way that this should still be reported as unpaid. The agency continues to report inaccurate negative information.
