February 10, 2017 - I filled out a form of cancellation for a - located in -, Washington and they misfiled it back in late-/-/-/early-/-/2016. The debit card connected to membership, around the same time of my membership cancellation, got lost so I cancelled it as well. I did n't inform them of the card cancellation because I thought I had cancelled my membership effectively. November 13, 2015 - In -/-/- of this year, me and my wife became aware that were are victims of identity theft. I have been actively disputing the ( - - outstanding collections that this company is reporting as my debt. Apparently, multiple checks were written to a grocery store -, WA back sometime between - - - - 2010. I have reached out to this company as well as my continued efforts of disputing these accounts as fraud - to no avail.
