May 23, 2016 - I requested that Apex Asset management LLC provide me with proof that I requested service from - - the debtor they are collecting for. -/-/2016 I requested " A signature '' " A contract '' something so I may see my handwriting or proof of some sort that this belongs to me. The first reply I got from them was nothing more than a print out with my name, the amount owed and pretty much the info that 's on my credit report. November 9, 2015 - I had a - in - - and was transported to the hospital by ambulance. I had medical insurance at the time but I did not have my insurance card at the time. Later, I called and provided my insurance information but the amount on the bill never changed. I called again but they insisted that I still owed the same amount regardless of insurance.
