Citibank Credit Card Complaint

My credit report show I have a credit card I have no knowledge of. I disputed the card with - and the dispute was resolved but the card remains. How is that? In the credit report there is a street address for the bank. There is no phone number of web contact. I have been removing these unannounced credit cards from my report. As I said, I have no knowledge of this card. I get no correspondence from them. I have not activated the card. I want the card gone. Can you help me. The bank is CITICARDS CBNA, - -, - -, SD -. I do n't understand what the advantage of these credit cards are. The are not used. They are not known about unless you track your credit report.
