Identity theft / Fraud / Embezzlement New Jersey

I received an unsolicited credit card account from Chase for an Amazon Visa credit card. It took a while but Chase 's fraud area did cancel it. Just to add evidence that it was a fraudulent action, the following day I received a letter from Chase saying my request for an Amazon Visa was denied due to 'duplicate request '.
