Unsolicited issuance of credit card Massachusetts

On -/-/2015 a credit card application for Kay jewelers was falsely submitted in my name - -. I found out about this from - which is a Identity theft protection service on -/-/2015. I did not submit any application to Kay jewelers for a credit card but yet it was done by them. I spoke to a manager in the credit card issuing department and I was told that there was no recent applications on me to apply for a credit card. I then called - and was told by them their was definitely a hard inquiry of my credit from sterling/ kay jewelers on -/-/2015. I would like someone to see why this happened and resolve this problem as well as having them take this false credit check of my credit report. I spoke with them about removing it as well and they proceeded to tell me they did not do the credit check. I - - give permission to pull my credit report for verification for this complaint, thank you
