Incorrect information on credit report Reinserted previously deleted info Pennsylvania

I had an account with - several years back that had been closed and now I get a open report back on my credit that states that 's on have an open account with them again that 's in collections this item was previously deleted off of my report I need help with this matter I 'm trying so hard to rebuild my credit to purchase a house this just made my score drop drasticly. I 'm trying to stay on the right path and do the right thing then this happens why are they reporting something that was charged off and closed once already.
