May 20, 2015 - On - -, 2015, after calling - Hospital to set up a payment plan for a bill for my children, I was referred to Optimum Outcomes by - from - billing to arrange a payment plan. I spoke with - at Optimum Outcomes. We discussed a payment plan for $60.00 per month but later that day I had $640.00 debited from my bank account. I did not authorize this amount to be debited.
