While gainfully employed by " -, i was assigned to pick up scrap metal battery racks from " - '', in - -. On -/-/-, while i was loading said battery racks, - racks that were stacked on a pallet fell upon my -. I was taken to " - - - '', were i received medical treatment for my -, for which defined said - as a - issue. On or about -/-/- i officially provided - - of " - of - of who i received medical treatment from in writing, for which was " - - -, - - - -, -, - -, phone # - totally $6700.00 and - - -, - Physician, - - -, -, - -, Phone - totaling $2200.00. Pursuant to the - self insured contract and settlement agreement and release, " - - - of - '', was responsible for paying the medical bills. The - contract nor was the settlement agreement and release was honored, for which defines a " Breach of Contract '', and a violation of Federal Law. The aforementioned has caused me significant hardship due to my not being able to obtain a loan to purchase a house and vehicle to start my own business.
