I was taken to " - - - '', were i received medical treatment for my -, for which defined said - as a - issue. Pursuant to the - self insured contract and settlement agreement and release, " - - - of - '', was responsible for paying the medical bills. The - contract nor was the settlement agreement and release was honored, for which defines a " Breach of Contract '', and a violation of Federal Law.
