Incorrect information on credit report Information is not mine Texas

There is a medical collection reporting on my credit report from a company I do not have any recollection of doing business with ( -. ) I have disputed multiple times with Equifax, only to have them reaffirm the account without providing any proof that the account belongs to me. For this reason, I want the account removed. I have disputed the account three times only to have Equifax update the account information, not provide me with information, and continue to mar my credit report. I am losing out on credit because of this, and this is my last resort to ask that it be removed off my reports before seeking legal remedies. It is not reporting on ANY OTHER CREDIT REPORTING AGENCY. Equifax has inserted this without proof it is mine, and continues to report a collection that is not mine on my credit report.
