Incorrect information on credit report Personal information Michigan

My problem is with eqifax they recently have started reporting wrong year of birth & wrong address. They sent me forms to correct but there forms are not for correcting year of birth nor address. Also they have no current report at all, it 's all old items, none of my current credit! Which makes it like I have absolutely no credit! All the other reporting agencies - & - are correct! Have tried to talk to someone and they keep telling me to fill out the dispute form, I keep telling them that their form is not formatted for this type of correction. Nor are the forms formatted for these corrections online. They outright refuse to correct it!
