I was sent an unsolicited credit or debit card from a company called " netSpend '', along with a direct deposit form " to give to my employer ''. The information in the letter I received contained active account numbers that were pre-generated " for me ''. I 've never contacted this company. I 've never filled out any forms requesting anything like this. I 'm not part of any program or institution that was acting on my behalf. This company just created accounts in my name without my permission and sent me the information.
