Unsolicited issuance of credit card Illinois

Company is reporting on my credit report that I owe them I just realized this is showing on the credit report I have written to them and they sent me back a identity theft affidavit and i asked that they investigate the debt but they are stating the form must be completed first but I have not indicated identity theft as of yet. I returned the form with notary because the police have not been contacted.I asked that a copy of any of the orders and or payments made be sent to me. no response as of yet
