The letter ( with a copy of the original application form ) was a follow up to correspondence from Chase which included a copy of a loan application with a number ending in -. The application also referenced and entity - ( - ) in connection with the same number ending in -. Our letter sent - -, made the following request : " Please provide the - number for the herein referenced APPLICATION for a Chase Home Equity Line of Credit. To date no acknowledgement or response has been received from Chase in regard to this request for information pursuant to RESPA 12 USC 2605 ( e ). Our correspondence was sent via US Postal Service with the tracking number - and was delivered according to the USPS website on - -, 2016 at - - in -, OH -.
