I was an emergency room patient at Presence - -, -, IL from - -, - through - -, -. I, allegedly, was seen by a doctor belonging to the - - - of - -, -. On - -, -, I started receiving phone calls from Escallate , LLC, a debt collector apparently collecting for - of - -, -. I verified my home address with them, and they agreed to send a letter of verification of the debt. To this date, I have not received any verification of the debt, any written communication what-so-ever from the collection agency, or any statements or written communication from - of - -, -. The collection agency broke numerous provisions of the FDCPA in attempting to collect this debt, which are outlined in the attached letter. After allowing the agency 30 days to send the notification they promised, I drafted a letter to the agency. On - -, I mailed a Certified Letter to Escallate , LLC, outing the laws that they broke and requesting that they cancel the account. I never received a response to this letter, which is also attached. Lastly, I was notified that they placed the account on my credit report in -, -, which means they violated further laws by failing to verify the debt prior to it being placed on my credit report. They are also reporting the Date of First Default as -, -. Having been seen on - -, -, the Date of First Default has to be at least - -, -, under provisions set forth in the FCRA. It is my firm belief that the collection agency engaged in deceiving practices by promising to send me written verification of the date and failing to do so, thereby exhausting the 30 days consumers are allowed to dispute a debt prior to it being reported to the credit bureaus. They have violated multiple provisions of the FDCPA FCRA, and 501 ( r ),
