In - 2014 I went to - - Hospital after receiving an -. I did not have my insurance card on me at the time but I called in the following week with the information. I was insured and have been insured for the last 10 years with - -. I later learned - - hospital sent my bill of $2600.00 to Capio Partners which is a debt collection agency. I have made multiple attempts to contact Capio partners with my insurance information and contacted health net to see why the debt is in collections. All attempts were unsuccessful and all parties have failed to resolve the issue. Although my plan has changed with health net we were still covered at the time of the incident.
