U.S. Bancorp Credit card department,

Identity theft / Fraud / Embezzlement Nevada

Someone opened a U.S. Bank credit card in my name, fraudulently, at some time on -/-/2012, without my knowledge or consent. Equifax did delete this account, but it was reinserted for some unknown reason. I did not open this account and I do not know who did. I 've faxed a copy of the - - - Police Department 's Police Report along with a notarized I.D . Theft Affidavit to the bureaus and have attached them here as well. After repeated requests, the bureaus & U.S. Bank refused to provide me with any information obtained from their investigation and with anything regarding their rationale for reinserting the credit item. I need the credit item being reported to the credit bureaus removed from my credit file immediately. I look forward to your response,
