Credit reporting company's investigation Problem with statement of dispute District of Columbia

My account with - - is paid completely and closed in - of 2013. As of now the account sits on the credit report as in collection and a separate collection agency for the account is there too which is ruining my credit and score. I disputed the item with all - credit agencies but got no where. I am attaching all document for your information.
