Below is a request that FMC has failed to comply with : I hereby certify and confirm this to be a true and correct statement of ac-counting as received by your office -/-/-. Pay Date Payment Principal Balance-/-/- - - - - - - - - Charge-off - - - Payoff Offer received with discount andAdjustment mad to affect amount due for settlement - - - N/A - Final Balance - within - days approve or correct this statement. Any corrections here-to must be submitted in writing, under oath and under the penalty of perjury, and full commercial liability ( where otherwise not prohibited by law ). FMC was also served with an Affidavit of nonresponse. This is an action for damages brought from violations of the Fair Credit Reporting Act ( FCRA ) 15 U.S.C. 1681 et seq.
