Medical Disclosure verification of debt Not given enough info to verify debt North Carolina

I have attempted, for several years, to get Optimum Outcomes to provide me with written copies of the original medical bill and the Explanation of Benefits from my insurance company showing that I did not owe this account. I have disputed this account that is showing on my Credit report. I have contacted the hospital and their -, 3rd party billers and they all show that I do not owe them any thing. I have a copy of of the Explanation of Benefits that show that this account was settled by the medical office. I have been denied a mortgage loan because of this false collection account.
