Medical Disclosure verification of debt Right to dispute notice not received Ohio

After discovering a recent negative account added to my credit file, I contacted the Debt Recovery Solutions company located in - - inquiring about the account. I was informed it was a physician bill from - years ago. I challenged that I would owe a physician $630.00 as I have always had insurance and asked for proof. After - weeks, I called a - time stating I did not receive the requested information. The lady was very rude, overtalking me and I had to ask her to let me finish my sentences. I informed her that I would be researching the validity of the account and asked what the discount would be for me to pay it, if it is found valid. I was informed that $530.00 would be accepted. After contacting - - - - - - ( my insurance provider for the last - years ), they stated the physician, - -, never filed a claim for rendered services. The fact that their office did not file a claim to my insurance company, within - months of service as allowed, can not leave me liable for the fees charged. Furthermore, I never received a notice of collection prior to this appearing on my credit report.
