I recently received a Chase Amazon Visa card in the mail. I had not requested this. I contacted Chase Bank to have the account cancelled and the inquiry removed from my card. Chase was unable/unwilling to provide details on HOW the account was opened, but their fraud department indicated I would be contacted by an investigator shortly and the issue resolved within 30 days. It has been over a month, no contact has been received and the inquiry has not been removed from my credit. Furthermore, Chase still refuses to tell me how the account was opened in my name.
