Credit reporting company's investigation Inadequate help over the phone Florida

It seems every other month there is issues with all - credit agencies. My score now has dropped in just the past year. - points. The way I found out is a chapter - I filed in - and - - - - was part of the chapter - I filed back then. The case ended in - -, - and the - were reporting I had a balance. So I sent them all the report from the court that it was paid and closed to take it off. Now just in the past year I found ( - ) HARD Inquires which I never applied for anything.. ( - ) - from - - - ( - ) - from -, - - ) -from - - and - card, - - - - - NCS, Factual data/-. I contacted them all and they say they ca n't take them off with out a letter from each Inquire. I do n't know what to do it ruined my credit I am working hard to restore. I am sure it is Fraud!! What can I do to get this taken off all my reports.- points went down in 1 year.. Please I hope you can help do n't know what else to do. Like I said I tried contacting all - agencies and put a fraud alert on my reports for 5 years ...
