Identity theft / Fraud / Embezzlement Ohio

Someone tried to open a chase amazon visa in my name, I called chase after I got an inquiry alert from my credit monitoring service, notified them that it was fraud because I never applied for the card. I do n't have enough information to file a police report which I need to place a freeze on my credit reports. They will provide any information to me
